Book of Forms and Student Identification Sheet
If you have not already done so, could you please immediately send back your child's Book of Forms and the Student Identification sheet. The information contained in these sheets is very useful and many sheets are giving your permission for your child to participate fulling in his or her learning. Please make any corrections on your child's Identification sheet so that we have your most current contact information in case we need to reach you in an emergency.
